Actions for independent living and de-institutionalisation for the elderly

Project description

The project includes a series of infrastructural investments aimed at preventing the institutionalisation of the elderly by providing them with innovative housing and equipment. The aim is to enable the elderly to maintain an autonomous and independent life, with access to home services, which ensure continuity of care. The socio-medical care model will provide simplified pathways for access to services and multidimensional and integrated care.
One of the main strategies will be the strengthening of territorial domiciliary services, with the creation of networks covering groups of flats not integrated in a structure. This will allow the elderly to remain in their own area, receiving the services required to meet their needs.Alternatively, the Service Local Areas (Ambiti Territoriali di Servizio, ATS) may propose the gradual redevelopment of public residential facilities into groups of independent flats.
The use of home automation, telemedicine and remote monitoring elements will help to improve the effectiveness of the intervention, enabling constant supervision and timely assistance.Activities will involve 260 elderly beneficiaries through the following projects: “CO.abiti_AMO”, “Nuova Roma 3”, “Ristrutturiamo Casa Giada”, “Casa Vittoria”, “Stiamo insieme anche se..”.
